This clinic is purpose built specialist orthopaedic clinic with allied health services such as Active physiotherapy and Orthotics available onsite. Free Off -street parking is available in the back of the building. Access is through Meadow Crescent.

The consulting suites at SJOG Berwick are located on Ground level in the main building. Parking is available in the undercover hospital car park. Access is through Kangan drive.
Your First Appointment
For your first appointment, please arrive 5 to 10 minutes early. You will be asked to fill out a Patient Information form and sign the Privacy Policy form. Alternatively, you can download and fill out the form prior to your appointment. You can send the form via mail, email or bring it with you to your first appointment.
X-Rays and Scans
In addition to your referral letter, please BRING YOUR XRAYS AND SCANS WITH YOU TO YOUR APPOINTMENT. If you have been advised that your X-rays are online, please ensure that you have your patient ID number and the company name where images were taken. Please be advised that your imaging may not always be accessible online during your appointment. Due to privacy laws, online images are generally accessible only by the practitioner who has ordered the imaging. This may in turn delay your diagnosis and management pending repeat imaging and a second consultation.
